It is mean that something wrong with your GPU or video driver: NVIDIA GeForce 9500 GT [ver.314.22]
Try to reinstall and to update driver to good working version 258.96 WHQL (or 275.33 WHQL). It is old but it is the best for old GPU like GeForce 9500 GT.
